| 内容記述 | As Well known，the Long-term Care Insurance is administrated by the municipality，and care management is very important to make this institution works well. The core of care management is monitoring. In this reserch，what is the way that monitoring should be like has been inspected. We have developed a monitoring-sheet to wake the monitoring work effectively and efficiently. We have inspected the effectiveness of this monitoring-sheet with the help of care managers in the home care management office. As the result， it's very clear that the monitoring-sheet and the comfirmed items in this sheet are very effective. Therefore，to make the care management works more effectively and efficiently，putting this monitoring-sheet into practice more widely is a urgent subject to do. |
